At the top of the window is the text:
"Select the following database creation options:"

Below this text are three options, each with a check box.

The first option is Create Database.

The second option is Save as a Database Template. This option includes data 
fields labelled Name: and Description:

The third option is Generate Database Creation Scripts. This option includes a
data field labelled Destination Directory: and a Browse... button.

At the bottom of the window are five buttons: Cancel, Help, Back, Next, and 
Finish. Next is grayed out, because this is the final step.
